Summer Sports Physicals

PanCare will be at Vernon Middle School for sports physicals on June 5th & 6th.

There are several forms that each student will need to have completed before stepping on the bus to see the provider. These are located below this post. Do not use PanCare Intake from previous years as they have been updated. All forms must be completed, and every spot for a parent/guardian signature must be signed. The EL2 forms have 2 spots that require both the parent and student to sign (Pages 2 & 4). If the student comes to the bus and any part of the forms are not filled out correctly or signed by the parent/guardian, he/she may be asked to reschedule if there are available appointments. Students need a physical copy of the forms to give to the team on the bus.

The provider can only see approximately 50 students per day. The first appointment is scheduled to start at 8:45am and the last appointment scheduled for 2:00pm.

After the physical parents/guardians must keep up with the EL2 forms that the provider signs off on. Then, parents/guardians need to fill out the Authorization to Release Medical Information form and email to [email protected]. Once they receive this form, it can take up to 6 weeks for it to be processed and copies of the requested forms be sent via certified mail.
